Initial Visit
Comprehensive data collection, medical history intake and Assessment, physical, consultation by NP
Lab draws will be offered in the near future. At this visit we will discuss past medical history, medications, preventative measures, immunizations, diet, exercise and health screening, Blood pressure, pulse respiratory rate, temp, pulse ox, bmi will be checked at each visit. Narcotics will not be prescribed.

Well Woman Visit
Includes vaginal exam. (Note: internal exams will be in the presence of another practitioner due to legalities- will be offered in near future). Blood pressure, pulse respiratory rate, temp, pulse ox will be checked at each visit, go over self breast exams and how to perform. Focused assessment, review of systems, immunizations, preventative care screening, PAPs (every 3 years will be offered soon), skin assessment, contraception, menopause symptoms, height, weight, bmi, Narcotics will not be prescribed.

Well Man Visit
Focused assessment, review of systems, immunizations, preventative care screening, prostate exam (Digital rectal exam, internal exams will be in the presence of another practitioner due to legalities, offered in near future), Blood pressure, pulse respiratory rate, temp, pulse ox will be checked at each visit assessment, referral to specialists, erectile dysfunction, testosterone check, Narcotics will not be prescribed.

Home Visits
Home visits are for patients who cannot come to the office because they are elderly, they have a mental illness and cannot leave their homes, they are immunocompromised, or simply cannot get a ride to the office. Everything from initial visit will be included. They can have access to the NP, wellness practitioner by phone and online appointments, Blood pressure, pulse respiratory rate, temp, pulse ox will be checked at each visit. Narcotics will not be prescribed.
Price: $200 for initial visit, $75 for each additional visit
Chronic Disease Management
Initial Assessment, Diagnosis, and then Follow-Up Disease Management. Diabetes, hypertension (high blood pressure), dyslipidemia (high cholesterol), Obesity (weight management), hypo/hyperthyroidism, copd, asthma, anemia, to name a few, Blood pressure, pulse respiratory rate, temp, pulse ox will be checked at each visit. Follow-up:
Every 6 weeks, 3m, 6m, or 12m depending on the stability of the disease. This also depends on adherence to the treatment plan, Follow up labs, medication adjustment

Episodic Illness Visit
Treat all acute illnesses (ears, nose, throat, mouth, viral, bacterial infections, rashes, gastrointestinal illnesses, urinary tract infections, musculoskeletal injuries/issues.

Telehealth Visits
Acute issues cold, flu, allergic rhinitis, candidal stomatitis (oral thrush), conjuctivitis (bacterial, allergic, viral) minor skin issues, eczema, contact dermatitis, dysmenorrhea, gerd (gastroesophageal reflux disease) prescription refills, chronic joint pain, herpes labialis (cold sores), insect bites, and hives, diabetes management, Uti uncomplicated, lab reviews, ADHD follow up evals and CBT will be available via telehealth when requested. Doctors notes will be available during telehealth visits and will be an additional charge. Any issues that require an in person evaluation will not be evaluated during telehealth visits, no narcotics will be prescribed.

Coming 2026, IV hydration therapy for those times you are dehydrated due to acute illness like nausea and vomiting and diarrhea. We will offer IV hydration monitored by nurse and NP.

Initial assessment will be 1 to 2 hours with 1 hour sessions to follow.
Cognitive Behavioral Therapy is a form of psychotherapy that aims to reduce symptoms of various mental health conditions, primarily Anxiety disorders, PDD, depression, and PTSD. CBT helps to deal with overwhelming problems in a more positive way by breaking problems down into smaller parts. CBT can lead to significant improvement in functioning and quality of life. Grief counselling, Trauma counselling, Relationship counselling are also provided.
Price: $150 initial visit, follow up visits $75 per session
Reiki Treatment
Our clinic will offer alternative methods to healing; the reiki master will not put hands on you but rather heal your aura and surrounding energy. To feel zen and relaxed try this out. Reiki includes a form of healing where the practitioner uses universal health transferred through the palms in order to encourage emotional or physical healing (nurse practitioner will be present during reiki sessions due to legal reasons)

Acupressure
Acupressure is an alternative medicine technique. It is based on the concept of life energy. Applying pressure to certain areas of the body can stimulate nerves, reduce the sensation of pain, reduce levels of stress hormones and helps the free flow of energy.

Respite Services (ages 5-99) coming 2026!
If pick up and drop off is required price will be $125 and a minimum of 3 hours is required.
Meant for families who have special needs children/ teenagers/ adults who just need a moment to get out and do self-care or take care of their own appointments, Respite services will be offered to Amherstburg and Lasalle residents at the Amherstburg office location at scheduled times. For Windsor residents and county residents we recommend calling ahead to book. We request info regarding your loved one, whether they need medicine during respite time, an enteral feeding or toileting at time of booking.
Price: $30 (1 Hour), $65 (2 Hours), $95 (3 Hours with Pre-book), If pick up and drop off are required, the price is $125 with a 3 hour minimum.
Nutrition & Exercise Package
Your diet will be looked at in depth, with consideration given to allergies and allergy friendly diets with an exercise routine recommendations. NP will be available for pre exercise program assessment and checking blood work. BMI will also be calculated and Calorie cutting and sustaining will also be calculated.
This is meant for people who want to live a healthier lifestyle but don’t know where to start with food prep. Nutritious and delicious food will be prepared fresh or with the option of frozen available.
1 Week Food Plan will consist of 3 meals per day and one snack per day.
Price: $850 + HST (1 Week)
8 Week Food Plan is for 3 meals a day plus 1 snack per 3 days a week.
Will include an appointment once a week for the 8 weeks program with optional weigh in. Appointments will be 45-60 minutes each week.
